From First Call to Ongoing Care

Initial Contact

Response within 60 min

Call us on +27 (11) 555-0100 or submit an online request. A care coordinator will respond within 60 minutes during business hours.

You will speak directly with a qualified care coordinator — not a call centre. We will ask a few initial questions to understand the urgency and nature of the care needed.

Home Assessment

Scheduled within 24–48 hrs

A registered nurse visits the patient's home to conduct a comprehensive care assessment. This takes approximately 60–90 minutes.

The assessment covers medical history, mobility, medications, cognitive status, home environment safety, and family support. A written assessment report is produced.

Care Plan Development

Delivered within 24 hrs of assessment

Based on the assessment, a structured care plan is developed. This is shared with the patient and family for review and approval.

The care plan specifies: care frequency, tasks to be performed, nurse assignments, escalation protocols, and review schedule. Nothing begins without family sign-off.

Nurse Assignment

Confirmed before first visit

A vetted, qualified nurse is matched to the patient based on clinical needs, location, and availability.

We aim for consistency — the same nurse for each visit where possible. You will receive the nurse's name, qualifications, and SANC registration number before the first visit.

Care Delivery Begins

Visits per agreed schedule

Scheduled care visits begin according to the agreed care plan. All visits are logged and reported.

Visit logs are maintained electronically. Family members receive regular reports. Any changes in the patient's condition are escalated immediately.

Regular Reviews

Monthly or as needed

Care plans are reviewed regularly — or immediately if the patient's condition changes.

Reviews are conducted by a registered nurse. Families are involved in all review discussions. Care plans are updated as needs evolve.

How quickly can care start?
In most cases, care can commence within 24–48 hours of the home assessment, depending on the urgency and nurse availability.
Do I need a doctor's referral?
No. You can contact us directly. However, we will request medical history and may liaise with the patient's doctor as part of the assessment.
Can I change the assigned nurse?
Yes. If the assigned nurse is not the right fit, we will arrange a replacement. Patient and family comfort is a priority.
