By engaging the services of Lifeline Home Care (Pty) Ltd, you agree to these Terms and Conditions. These terms govern the relationship between Lifeline and the patient/family ('Client').
Lifeline provides home-based nursing and care services as described in the agreed care plan. Services are delivered by qualified nursing staff under clinical supervision. The scope of services is defined in the care plan and may be updated following a care review.
Cancellations must be made at least 24 hours before a scheduled visit. Cancellations made with less than 24 hours' notice may incur a cancellation fee. Emergency cancellations due to hospitalisation or medical emergency are exempt from cancellation fees.
Payment is due as per the agreed payment schedule. Lifeline accepts EFT, credit card, and medical aid (where applicable). Overdue accounts may result in suspension of services. A 30-day notice period is required to terminate ongoing care arrangements.
The Client is responsible for: providing accurate medical information; ensuring a safe working environment for nursing staff; notifying Lifeline of any changes in the patient's condition; and maintaining agreed payment schedules.
Lifeline is responsible for: providing qualified, vetted nursing staff; maintaining accurate care records; communicating changes in the patient's condition to the family; and maintaining professional indemnity insurance.
Lifeline's liability is limited to the value of services provided. Lifeline is not liable for: outcomes resulting from inaccurate information provided by the Client; events beyond our reasonable control; or conditions requiring hospital-level care that were not disclosed at the time of assessment.
Either party may terminate the care arrangement with 30 days' written notice. Lifeline reserves the right to terminate immediately in cases of: abusive behaviour towards staff; failure to maintain a safe working environment; or non-payment of fees.
These Terms and Conditions are governed by the laws of the Republic of South Africa. Any disputes will be resolved in the courts of South Africa.
